He leaves her to fall off the bed, goes to the door and says 'Do you want to finish her off?' to absolutely no-one. Naturally, she asks who he's talking to. It's still hard to visualise the 'real' situation when you know.
Actually I think it was Jack who decided it was time to meet Tyler, because if you think about it, Jack blows out the pilot light on his stove, then shatters his own lock, plants some home made dynamite, and THEN leaves on a business trip.
Or when Jack fights pretty blonde boy. Notice his mannerisms. They are directly from Tyler. How he smiles, how he moves.
(I would also add that he is being Tyler in the office when he is smoking 'You don't smoke', and look how much wider his eyes are than usual. He also has a quietly sarcastic bearing. Or I'm imagining it. Sometimes an actor can be too good.)
When the guy with the car pulls up and says "your car, Mr. Durden" - he's looking at Jack. Jack looks back at Tyler and says"after you Mr. Durden" and so on. The guy who drove up with the car shakes his head afterwards, wondering what's going on.
I think that everyone around him knows he has two distinct personalities. That is why Pretty boy stops him reading the files when he's pottering through the kitchen - because he recognises it's the 'wrong man' from his shuffling behaviour.
It's a disturbing sign of their worship that they still follow him despite this.
